Dr. Boyd will meet with you for a 30-minute hyperbaric oxygen therapy consultation.  In the appointment, she will review your paperwork, answer any questions you may have, and review your consents to clear you for therapy.  After your consultation, you will have the opportunity to receive your first 60-minute hyperbaric chamber treatment.   In order to schedule a hyperbaric oxygen therapy consultation, you will need to complete the paperwork listed below.  We will call you to schedule your 30-minute consultation and hour long chamber session.

Prior to scheduling an appointment, you will need to do the following:

• Complete the form by typing right onto the forms
• Be sure to SAVE the forms to your hard drive, otherwise you will lose what you typed
• Create an email to info@forwardhealthsolutions.com and attach the forms you saved, OR
• Print and fax to 601-450-2079
